Whether an elder lives near or far from family resources, a Stubbe GCM can effectively oversee the entire care giving process. First we conduct a comprehensive initial assessment to determine what services are needed. From there we develop a plan, connect with the appropriate services and monitor the care process.

These services may include, but are not limited to:

• Assist with scheduling appointments
• Assist with transportation for appointments
• Attend appointments and serve as the connection with family members and professionals in the care network
• Assist with medication set-ups
